Unfortunately, at this time, we are not in-network with any OHP or Medicare advantage plans.

Once you schedule your initial visit, we will check your benefits for you, however it is the patient's responsibility to know and keep track of their own benefits.

If we will be billing your insurance, please send a copy of the front and back of your insurance card along with your address and date of birth to contact@alignandshinepdx.com

Pricing

If you will not be using insurance, we offer a discounted rate through Chirohealth USA for Chiropractic care. New patient exams and treatments are $139. Returning patient treatments are $65.

Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed appointment leaves a hole in the providers day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be automatically charged a cancellation fee of $50 as we are unable to bill insurance for missed appointments.

Patients that are more than 15 minutes late are considered a no show and will be charged a cancellation fee.
